ACTUARIAL AND FISCAL OPERATIONS REPORTS
These records consist of actuarial reports and work papers for Worker's Comp, Auto/General Liability, Education Legal, and Medical Malpractice. They also include fiscal reports created annually for the Office of Risk and Insurance.
AUTO, ATHLETIC ACCIDENT, GENERAL LIABILITY AND WORKERS COMPENSATION CLAIMS
These records are the "official" claim files and include all paper and electronic copies of medical forms, medical reports and bills, proof of loss, reports, invoices, pictures, recorded statements, financial information, adjuster’s notes, settlement notes, investigative materials and any other relevant documents. This category also includes Worker’s Compensation Incident Reports where no claim was filed.
BANK STATEMENTS AND RECONCILIATIONS
This group of records contains quarterly bank statements on Medical Malpractice (MMP) trust funds and reconciliation sheets between the trust funds and claim system.
BENCHMARKING REPORTS
These records include Advisen Benchmarking Study (work papers), Midwest Higher Education Consortium (MHEC) Benchmarking Study, Worker's Compensation Study, and Worker's Compensation CAT Study.
CLAIMS AND PROCEEDINGS - EDUCATOR'S LEGAL LIABILITY, POLICY LIABILITY, CYBERSECURITY/DATA BREACH, MEDICAL MALPRACTICE AND ADMINISTRATIVE PROCEEDINGS
These records are supporting files with the "official" copy of claims and proceedings held in the General Counsel's office. These records may include but are not limited to medical reports and bills, completed state forms, investigative materials, invoices, recorded statements, financial information, adjuster’s notes, settlement notes and other supporting documentation.
FILE HANDLER DOCUMENTATION
These records contain development documentation of the File Handler Claim System and includes working papers dealing with the software implementation and subsequent modifications, testing, and problem resolution records.
INCIDENT REPORTS - NO CLAIM FILED; DENTAL MALPRACTICE, AUTO LIABILITY, GENERAL LIABILITY AND ATHLETIC ACCIDENTS
These records are the "original" reports filed for incidents only where no dental medical malpractice, auto liability, general liability, or athletic accident claim was opened.
INCIDENT REPORTS - NO CLAIM FILED; THEFT AND DAMAGE REPORTS
This group of records contains theft and damage reports to Student, General Public, or University Property that are submitted, but are considered incidents only, or University property below the deductible, or items not covered such as personal items or currency, checks, etc.
INSURANCE POLICIES - COMMERCIAL LIABILITY COVERAGE (TYPE A)
The following commercial liability coverage policies are to be kept indefinitely: Athletic Accident, Auto General Liability, Aviation Owned and Non-Owned, Cybersecurity/Data Breach Protection, Educator’s Legal Liability, Errors and Omissions, Excess Liability, Experiential Student Liability, Fidelity Crime, Fine Arts, Foreign Liability, Helipad Liability, Lawyers Professional Liability, Managed Care E&O, Medical Malpractice, Nuclear Engineering Liability, Product Liability, Property Liability, Workers Compensation, and Workers Compensation Excess.
INSURANCE POLICIES - INDIVIDUAL OR EVENT BASED COVERAGE (TYPE B)
The following policies are to be retained for 10 years: Air Ambulance, Camps and Clinics, Child Day Care, Crop Insurance, Disability Policies, Event Cancellation, Fire Trainer, Hole in One, Inland Marine, Livestock Risk, MPST Insurance, Special Risk, Sports Club, Student Health, Study Abroad, Travel Accident, UHC Global Med Ex, and Venue Insurance.
MEDICAL MALPACTICE (MMP) NATIONAL PRACTITIONERS' DATA BANK AND MEDICAL MALPRACTICE STATE QUARTERLY REPORTS
This group of records contains reports on Medical Malpractice (MMP) settlements paid by the University filed with the National Practitioners Data Bank and correspondence with state licensing board as well as claims and status filed quarterly with the State of Missouri.
MEDICAL MALPRACTICE CLAIMS HISTORY LETTERS
Records in this series consists of issued history letters created by Risk & Insurance Management.
PROPERTY CLAIMS
These records are the official property claim files and may include, but are not limited to proof of loss, reports, invoices, pictures, recorded statements, claim financial information, adjuster’s notes, settlement notes, etc.
VEHICLE INVENTORY FOR INSURANCE COVERAGE
This records series consists of annual vehicle inventory and vehicle updates by campus and are used for insurance coverage only.
WORKERS COMPENSATION TAXES, ANNUAL REPORTS, EXPERIENCE MODIFICATION, MISCELLANEOUS
These records include Worker’s Compensation (WC) tax forms filed with the State of MO (quarterly estimated taxes and annual Table 1 filing), Annual Reports required by the state to remain a self-insured entity for WC, papers submitted to National Council on Compensation Insurance (NCCI) and correspondence in calculation of WC Experience MOD. Also includes correspondence with State of MO estimated payroll, audited payroll, correspondence, deposit and earned premium.
