What is the Health Risk Assessment?
The health risk assessment is an online questionnaire designed to identify where you are making healthy decisions and areas that you can target for improvement.

Why should I take it?
The health assessment helps you identify your health needs. Collectively the data from the assessments will be used to help Healthy for Life determine the university’s program needs. Our staff will not see your personal information and your information will not be shared with anyone at the university.

How to take the Health Risk Assessment

1. If you are a Coventry enrollee, log on to www.ummedcvty.com
2. Log or register for “My Online Service”
3. Click on "Wellness Tools"
